Looking at mine with the dash out I think dirt gets in and makes the doors hard to move and that's what kills the actuators. A lot of them have cracked and broken gears from the stress.

Well your top picture is closer to what mine did. That metal piece came off the shaft and was still connected to the arm with the clip. I took my ashtray out and was able to get that pushed back on but. It seems to be working but I did not realize it was D shaped. I must have got lucky as it pushed right on and now I have defrost again. Now you got me thinking did I get it indexed right on that shaft. Hmmm...
