Not so much a custom tool as a custom use of an existing tool.
See first two pics of spanner in very cramped (smart) engine bay and unable to exert enough force on it or slip something over it for more leverage due to proximity of small plastic fragile pipes on end of inlet manifold.
So, to gain more leverage I took a large adjustable spanner and placed it as in the photos below and used the far away end as a pivot point against the forward firewall and pushing down on the jaws end with the main part pressing down on the small spanner I got enough force to slacken the bolt.
Might be handy when working in cramped areas, using whatever's cramping you as part of the mechanism.
