I’d cut it out personally
Then roofs are double layered... I’d pull the windshield cut out all the rust find another cab and cut out more than you need then butt weld it in prime from the inside if I could ( that’s hard I know ) then I wouldn’t have to worry about it coming back..
imagine spending all this time and money for it to just come right back through the paint
Try removing it all first, like you said that doesn't look extreme.
