Can anyone tell me if these wheels will fit my 92 K1400 extended cab? I appreciate it.

Without the measurements, no. I have these same rims made by GM on my 1997 k1500 in the 20" size and they fit.

Yes. GMs bolt pattern is the same. I have seen the top rims on several GMT 400s. The bottom rim, IDK, but if its GM truck rims, them yes.

No idea, but I like the center cap.... looks like it came from a 70's truck.

As long as they're not replicas, they should fit. I've got 33" tires on mine, they rub a little on the inside of the frame when turned fully, but not enough to cause any significant wear. Could be avoided by going lightly narrower, or slightly shorter.
