The lens is from 2nd gen S10 blazers. It fits the sunglasses spot the best, but also fits the garage door opener. It will have a gap on each side because of the curve in the console towards the front in the garage door opener spot, but no gap if it's in the sunglasses compartment.

OP, search eBay for "12v led board" and pick an led board that you like.
