Looks to me like I'm going mad rich when the problem happens, assuming of misfires I stuck small chunks of wire under each spark plug boot on the distributor, and wile I had someone power break it, grounded each one out one at a time. Each time i can tell the engine was loosing a cylinder, so it isn't a misfire...

yea your BLM is trying to pull fuel to the max. if your fuel pressure is good (13-14 psi) i would look at getting injectors professionally cleaned or replace them.
make sure to verify both injectors are firing first. and check the part numbers to make sure no funny business.

I replaced the entire throttle body from a known good truck, with zero change in how it acts, its from the same year same size engine, only difference is it was from a gmc half ton

Humor me, what would a warn cam/lifters act like? Thinking maybe the extra fuel is from not enough air, say my intake valves are not opening as wide as they should... idk, is it worth pulling the intake to have a peek at the lifters? It revs up just fine in park or neutral, will red line and come right back to an idle... how would I go about diagnosing that?

do a compression test
Already done that, that won't tell me if the cams warn, that tells me the valve are sealing, but if it doesn't open far enough to alow enough air through is where my heads at...

is your firing order correct and are all cylinders firing? can you smell gas out the tail pipe? if your exhaust smells "rich" it usually means raw fuel which. is incomplete combustion. I am not to sure what your skill level is, usually you can hear if a cam is worn that badly or bad lifters. another easy check is to roll the engine over by hand and see what the valve lift is if you're worried about a worn cam.

Agree, Not every cam fail is catastrophic.

i have run plenty of late 70s-early 80s 267/305/350 that had "soft" cams. They were just lame, but still ran. ie: not incomplete combustion causing "rich" condition

EDIT: I am going to add that people often mistake gasey exhaust as rich. it isn't rich it's raw fuel. usually when the ECU is pulling max fuel it's because there is something up with fuel delivery or ignition.
