I have a 92 c1500 that has no constant 12v to the cab. I had to run a wire for the stereo constant to the 12v block on the firewall, replaced all bulbs and plunger for courtesy lights and they only work with the key in accessory. I've checked all fuses under dash on driver side. Any ideas what it could be?

Sounds like a blown fusible link. Since your ignition switch still works, you're getting constant 12v there, so I'd take a look at the wiring in the junction block on the passenger side firewall of the engine bay. There's a black fusible link that supplies power to the Ctsy fuse (runs the interior lights, radio, park lights...) that's probably going to be your culprit. See the right side of this image.
