U-Bolts need to be re-torqued after about 100KM or so, i used heavy duty custom made ones on my '96 (Uncle owns a spring shop), and mine loosened right off just by going back and forth to work a couple times.
Without seeing or knowing what happened, hard to say, U-bolt's don't really "fail" other then breaking.
I have bought 3 Canuck Motorsport's lift's and used 2 of them (my 2" i decided not to use), my '96 has their 4" spindles and my ex's '07 has a 3" leveling kit I installed, and last time I saw the truck it still had it.
But with that being said, never have used or seen there u-bolt's
