Not quite what you are looking for but I have an Extended Cab Short bed (ECSB) 2wd with a 305. I get about 17 in city and 19 Highway.
Common factor is that I am also in NM! :)

Manuals do get better until you put bigger tires on. If you stick with OEM or slightly bigger 245-265/75-16 you'll be good at around 17-19 like said above. You could get in the 20s if you draft a semi on long trips.

Yea, I guess I didnt make that clear, with larger tires like 295/75 or 305/70 and a little 305 you need to keep it in 4th 95% of the time which negates the benefit of a 5-speed. 4l60's can still wonder but they go into OD more often.
