Non contact thermometers are handy when troubleshooting. Check the thermostat housing temp is a good way to check a lot with no effort.

How much of a difference does your gauge read compared to this:

Almost identical
That's exactly where my gauge sits when it's warmed up. Climbing a steep hill, idling, coasting down a steep hill; it never budges.
As for your smoke/steam; does it smell like coolant? The heater hose bypass is famous for leaking, or it could be a bad rad cap. It could also be a bad valve cover gasket leaking oil on to the manifold.
