And the pistons have nothing to do with the afm/dod unless it's so far outta time the valves hit but that wouldn't be due to afm cam.
The lh6 is kinda odd so better know what year it is.
2005 trailblazer ext had afm/dod in a lh6 aluminum block 5.3 and was still the gen 3 24x crank and 1x cam but its in timing cover. The block is gen 4 so kinda a odd deal
