^ seems to be the winner. Truck finally acted up for a shop and they tracked it down to cap and rotor. Although every now and then she stumbles at start (had the press the throttle to start at one point and ran rough for a few seconds), almost like there's water in the fuel... running a bottle...
So appears that there was some corrosion/"carbon trails" in the distributor- new cap and rotor has her firing up in proper time! Next fun issue that's shown up is where she would stumble a little at first still presents itself- to the point I actually had to feather the gas pedal to even get her...
Still randomly happens after at least a couple days of sitting, can't reproduce it consistently to really diagnose. Have a coolant temp sensor on the way to throw at it
Would checking those be dependent on actually catching it while it struggles, or do you think they'd show themselves any time? I haven't figured out how to replicate it quite yet... hit or miss after a few days.
Also, on occassion it seems to stumble a bit on startup- just for a couple seconds...

Will let you know- I'm almost mobile enough to get started on this. I will say that my issue seems a little opposite of yours- once it's warm it tends to act better at least, issues come up with sitting for "short" (within a week) periods of time
