Does anyone know if a 98 steering wheel can be swapped into a 95?

Here's one from an Escalade I found in an eBay auction:

Any '99-'02 truck wheel with audio controls will work. Some Blazers and Jimmys even have wheels that work.

Yes. You will need a new steering wheel, airbag and clockspring. I swapped a 98+ steering wheel with audio controls into a 95 a year ago.

Do you know where I can find instructions for this swap?

I did a writeup on GMFS and FSC, I just haven't had time to re-write it for this forum since finals week has been killing me. I'll get around to it later this weekend.

If you get a wheel from a 01-02 Denali or 02 Escalade, it will work but the steering wheel, airbag and the trim behind the buttons will be the wrong color. If you find one from a S-10 blazer/jimmy/bravada it will be the correct grey color.

The controls are available separate and will work on any square airbaged steering wheel. Then you'll just need the correct trim and clockspring.
