95 tbi 350 surge/stumble

Recently my 95 Sierra started to act up on me. When accelerating, especially in 2nd and 3rd gear (standard shift) it has a surging/ stumbling feel to it. It also has had a loss of power it feels and also reduced fuel economy. This happens cold and hot but more so with the engine colder. What could this be? It's driving me crazy and I drive it 100+ miles a day and can't afford 12mpg on the hwy when I was getting 16-17.5
