I found a new set of center caps for my 15" Rally wheels on my '96.
There is an eBay seller (trailerpartsking) selling a 4-pack of chromed plastic trailer center caps that happen to fit the 5-on-5 pattern. I figured for the price ($49.99 shipped), why not try 'em?
They look fantastic and really enhance the wheels. The install might be a little easier with short (non-external-thread) lug nuts, but I'm very pleased with how snugly they do fit.
I'll get a better pic after I get Bowtie washed, but here's what they look like.
Still on the fence whether I'll put emblems on them ... I actually kinda like 'em without.
By the way, these are made by Phoenix USA and the part number is #QT550CH4. You can find them at many trailer parts stores and online, too.
EDIT: Here's a full view of the driver's side with 'em on. Still need to wash the truck, but I wiped the wheels down.
