rear disc swap 8 lug c-clip rear

new guy here. i have a 99 chevy k2500 (old style) 4x4 and would love to do a rear disc swap. is there any one that makes a kit or is there a factory set up that can be fitted to it/ it has a 14 bolt 9.5 c-clip rear end in it. i knew a guy that swapped in an 02 2500 hd rear into a 97 and had the spring perches moved. the prop. valve would have to be changed also. thanks in advance, paul c.

i saw that, thanks. i will have to use the tsm kit since i have 8 lug axles.
