Belt tension rattle

My belt tension has this rattle.ive replaced it with a new one and it dose the same thing. Any ideas? Bent pulley from somewhere else

Alternators make a rattle when the bearings start to go. It is hard to localize the noise sometimes I would remove the belt and hand spin all pulleys to see if any are making noise.

U can visual see the actually tensioner making noise.since it's spring loaded it it rattles so I'm thinking i might have a bent pulley on one of the accessories

My 90 Sierra 5.7L rattles in the same spot. New pulley. I took a small piece of cardboard and put it in the space and no more rattle. May not fix it long term but it's lasted 3 years so far.
