Ladders. Honestly though, all of the swamp trucks and huge Sema trucks they use ladders to get into.

I don't know why, but I actually like it.... from that view- from the front or the rear it probably looks a little top heavy. Most of those trucks do because you can't get the stance you can with a reasonable lift.

jungle gym method-grab door sill grab part of door, use upperbody streght to lift and bring legs forward to climb up tire, move hand to a pillar and seat areas, and jump backwards into truck.
