I've noticed recently that my Suburban is starting to tick under load or acceleration.

Idle or cruise there is no ticking noise.

Ive looked underneath to see if i might have a slight exhaust leak. Nope solid.

I do have a lot of miles 237k, and I was hoping to keep on trucking till 250k+
Any idea what that sound might be or concerns I should have?

Did you check the exhaust manifolds? My dad's 2003 1500hd 6.0 developed similar symptoms...replaced an exhaust manifold and the tick went away.

Mine did this and it was one of the manifold gaskets that had a chunk blown out.

Driver side down pipe cracked at the flange on mine. Exhaust guy said it was common for our trucks.

If you have some really long needle nose pliers, hold a little piece of a paper towel with them and use that to kind of probe around all the exhaust items. A leak will blow that little piece of paper towel and indicate where your leak is.
