That's the "Sport" grille. I would call LMC truck.

I had a chrome version of this grille on my truck. Sold it and bought an OE paintable one because I wanted to change the look. Actually fit decent for an aftermarket grille. They seemed to be everywhere on eBay at the time?

I want to say these were made by Street Scene, in chrome and black varieties.

Haven't seen them around for sale anywhere in a long time, and no evidence of them on Street Scene's site. I'm guessing they're probably discontinued.

It’s a Street Scene style grille. They’re hard to find and expensive if you do find one. My truck came with one already on it, but I need to find a new one because the chrome isn’t peeling, but just doesn’t look good

You'd be better off repainting or vinyl wrapping your current grill chrome
