316k miles and it misfired before new plugs, wires and all that went in... Have you done a compression test? The original engine in my '89 would foul out the plug in cylinder #1. Before it would totally foul out, it would miss under load on the expressway. Come to find that cylinder had burned up the exhaust valve. Maybe you've got something like that going on. When you put new ignition components on it, did you set & verify base timing was at 0 degrees?

Code 43 could be caused by someone changing the starter out and taking the single wire going to the knock sensor with it. A scan tool would allow you to see if the truck is recording knock events or not. If the truck doesn't see knock for a while, it will try to self test by really advancing the timing. If it doesn't see knock after that, it sets the SES light and reverts to base timing.
