They're 19.5" x 6" Alcoa aluminum wheels made for 80's-early 90's GM motor homes. They share the same bolt pattern as the 1991-2002 3500HD trucks - 10 lug x 7.25" bolt circle. Not easy to find.

My good ol' 2000 C3500 still has the stock 16" steel wheels. I recently bought 5 new tires for it at Costco (the original spare didn't look too healthy), and while the tire tech was installing them, he showed me that the backs of the plastic hub caps are all cracking around the bolt holes. That style hub cap seem to be yet another pricey thing to find new:

Rather than spending about $400 on a set of new hubcaps, maybe I should shop for a set of new wheels instead. Any suggestions on a nice stock-size 8-lug wheel? Since I spent about $1,000 on the five new Michelins, I'd rather keep 'em for now.

Of course buying used hub caps on eBay is always an option, I found chrome ones going for $19.99 each, but who knows how brittle they are and how long it'd take for them to start cracking like mine?

There's a set of four of the OEM GM black hub caps like the ones in your picture up on eBay for $32 each right now, brand new. Just FYI. I really like the stock wheels and caps on these trucks.

I like the stock look as well. As I was telling the Costco tire tech, it's too bad that I couldn't get him to paint my wheels while the tires were off. The wheel in the photo above shows some missing paint, the others show a bit more. As for the NOS hub caps on eBay, if I got a set of those, then I'd probably need a set of bowtie emblem inserts as well, since I doubt that I'd be able to peel off the old ones undamaged. They ain't cheap new, either:

Here's one new one that's on eBay for $16:

Polished the raw aluminium wheel lips the other day with some Wenol metal polish. Took about 3 or 4 minutes with the buffing wheel on a drill motor.....
Now I'll have to do the whole wheel .....
