I wonder if the 12K C&C came in 4wd? If so that would explain the longer chassis and the narrow dually axle.

I'm sure it did, since it was essentially a "regular" 3500 but with the rear frame difference (parallel 34" rails) and longer wheelbase. The rest of the truck from rear cab wall forward is the same as your typical 3500.
That truck does indeed look like someone took a C&C 3500 with the 159.5" wheelbase, roughly 2' and a couple inches longer than a regular truck, and custom made a longbed for it.

10 ft beds were available for fleet purchases. Not real common. If it is part of a fleet purchase that could also explain the narrow dually rear. You would to see the RPO codes to verify this.
That would be a stretch!!! Lol, actually, that’s pretty cool.

From what I understand, 12k= 4wd and 11k was 2wd.

I know Ford had them, but i've never heard of GM doing it. Any idea what the RPO is?

Don't know the RPO code. I knew a guy that had one. They also had a truck called the Long Foot 12 ft bed!

Those almost look like someone modified the images. Not that I don't believe it. Back in the early 70s gm also had the longhorn 9ft bed trucks. I ran across a few of those over the years but that is the first 400 I have seen with an extended bed.
