88 chevy c1500 engine noise

my truck has developed an engine noise (at least it seems to be coming from the engine). It sort of sounds like a coin being shaken in a can. (link to video below). You mostly hear it towards the end of the video, but what could this be? is it a problem? the sound doesnt happen in park. only when the truck is in drive or reverse.
