The early switches are hard to find unless you want to pay big bucks on ebay. Here's some info for you though. As someone posted you can use the defrost switch pigtails for the fog lamp switch.

These come out the front, there are clips on each side. They are easier to remove without breaking if you remove the whole bezel though. Get the plugs for them while your at it, a lot of people like myself add these switches to trucks that did not originally come with them.

I had the same problem awhile back. I replaced everything, and still no luck. It turned out to be the brake hose had collapsed internally like 98_k1500 had previously mentioned. Even though it looked fine and did not have leaks, it was the culprit. replaced the hose and all was good.
