Maxed out fuel trims for no reason??

In serious need of help here. I recently bought a 98 sierra with 5.7 vortec at 340xxx km.
On a good tank I'll get 200km on a full tank and no more. Occasionally it throws the odd lean code however me nor anyone else is able to figure out why. Upon hooking up a laptop and checking everything out we realized the long and short term fuel trims are maxed right out on both banks. I tried replacing one sensor out of curiosity and nothing changed, fuel trims still maxed right out. I also replaced the maf sensor and nothing. Injectors all seem to check out, good steady fuel pressure and vaccum is also good. Had a shop replace the entire exhaust system to rule out any leaks causing a lean condition can anyone suggest what else to look for?

The gauge seems to show the thermostat opening but if it was the coolant sensor would it not reflect on the gauge?

Actually come to think of it we checked out the ecm's coolant reading and it matched perfectly to what the gauge was reading

Check out videos by "South Main Auto" on youtube. He's got a couple where the vehicles did that and the troubleshooting that goes with it. Might help.
