New TBI for '93, 5.7?

Would I benefit much by installing a new tbi in my 245,000 mile engine? Recommendations on what to install?

^ X2, Unless there's something wrong with the one you have I wouldn't worry about it.

What is wrong with the one that is on the truck now?
Nothing wrong to my knowledge. Is your take (& df2x4) that a tbi can and perhaps should continue to work more than adequately after 27 years of use and there would only be a minimum amount of improvement gained by replacing it with a new one.

I originally wanted to replace the cam. But I have been advised that I am opening a can of worms to do so on such an old engine.

I have a '90 K1500 with around 160,000miles on it and it has the original TB on it. I also have a '92 K 1500 with 263,000 miles on it and it has the original TB on it. My '89 G10 van had the original TB on it when I scrapped it due to rust and I have that engine sitting in my shop ( it still runs). My DD is a '98 Suburban K 1500 with over 255,000 miles on it and it is working just fine. None of these TBI units are worn to the point that they cause any issues.

I guess my point is that UNLESS the TBI unit is worn and is causing drivability issues Run it until it does cause a problem.

To be honest, there's really nothing to ware out in the TBI. What you can do in rebuild/clean it. There are lots of little veins at the base inside that get gunked up over time, as well as the little screens on the injectors. A rebuild kit includes all the gaskets, O-rings, and the injector screens. The pressure regulator diaphragm is a bit of a bear to install, but nothing serious. Put it back together with some anti seize for all the screws. The TBI is aluminum.

The TBI computer can handle a mild cam upgrade (.510 and 112-114 LSA) , or a 383 upgrade just fine. You run into problems with thumper cams.

I have seen them wear the throttle shafts to the point of hanging the idle at about 1,200 rpm. Also seen the fuel pressure regulator spring fall apart more than once.
