Would I benefit much by installing a new tbi in my 245,000 mile engine? Recommendations on what to install?

Nothing wrong to my knowledge. Is your take (& df2x4) that a tbi can and perhaps should continue to work more than adequately after 27 years of use and there would only be a minimum amount of improvement gained by replacing it with a new one.What is wrong with the one that is on the truck now?

I originally wanted to replace the cam. But I have been advised that I am opening a can of worms to do so on such an old engine
To be honest, there's really nothing to ware out in the TBI. What you can do in rebuild/clean it. There are lots of little veins at the base inside that get gunked up over time, as well as the little screens on the injectors. A rebuild kit includes all the gaskets, O-rings, and the injector screens. The pressure regulator diaphragm is a bit of a bear to install, but nothing serious. Put it back together with some anti seize for all the screws. The TBI is aluminum.
The TBI computer can handle a mild cam upgrade (.510 and 112-114 LSA) , or a 383 upgrade just fine. You run into problems with thumper cams.
