I thought the same thing as you regarding the 2500, 8-lug kit. I'm guessing the frame is taller on those which makes complete sense. This is definatly a fail on Belltech's part. Box says p/n 6650, instructions say 6605-6607, and then they give me a notch for a completely different truck...
Edit: I found this, which made me laugh because that is exactly what my truck would look like if I installed the notch.

So I'm about to install Belltech 6650 flip kit on my 92 454ss... I took the bed off and lined up the notch to the frame and there is a like a 1/2 gap - meaning the frame measures close to 4.5" and the notch measures 5" ??!

FWIW, I had the exact problem as you on my 92. I originally thought the problem was in the TCC because at cruise the TCC would constantly engage/disengage. After I swapped plugs/cap/rotor/wires the problem still existed. It turned out to be that I had lots of slop in the distributor. Ended up...
