Alright, update: Nothing loose externally, no crank walk. I think it’s the C-Clip in the Transfer case that’s notorious for breaking. Ordered the kit and I’m gonna to pull it apart this weekend.

You'd think that if it was the transfer case, it'd do it in any gear, not just first.

When the C clip in the transfer case breaks, it allows the output shaft to shift enough to cause the gears to grind, this comes from driveshaft movement. I’d imagine my old 5.7 only has enough power/torque in first to shift the driveshaft around that much I also have leaf helpers so zero axle wrap.

Is it possible it could effect reverse? My 97 K2500 jumps out of reverse and grinds when under a load (heavy load, backing up a hill, backing over a curb, body twisting in reverse)

And can the Clip/output shaft movement be detected with the truck up on a lift?

How involved is that repair? Have to "gut" the transfer case, or merely replace the broken clip after removing the tailshaft housing?
As simple as chocking the tires, removing the driveshaft, and the tail shaft housing from the transfer case. I ordered the sonnax repair kit as it’s beefier and a one time job. You will need some RTV to seal up the tail shaft housing and some transfer case fluid to top it off as some will drain when you pull it apart.
