(SOLVED -ish?) HELP! Lug nut thread pattern shenanigans

I keep waiting to see more pictures of the underside..... front end same lugs? Allison TH400? None of that makes any sense, to me.
Yeah, the deeper i go, the more i wonder what the hell i have... Instead of posting pictures all day, i'll attach a few and give you guys a video.
I'm not the savviest mechanic, and i also have been on two wheels only since i moved to Cali 3 years ago, so i'm going off of the little i know of my old 88 c2500 from years past, and honestly, the whole of that lug shank discussion was in a foreign language. It's VERY possible this is not right, and i don't know better to catch it. Also, never even drove a dually before bringing this home, until i took this pic, you could've said dually wheels were one solid cast rim that accepted two tires. Learning as i go here, so thank you guys for dropping the knowledge. Youtube video linked.
And if anyone can help me narrow down the gear ratio that would be awesome

Went looking for a bill of materials tag because the number that matches up with the Ford van bill of materials is not actually on a tag, it's just stamped on the back of the cover. I only found one tag of any kind, and at first I thought it was just the nub of one broke off, but if were a gambler I'd say this is the gear ratio tag

Eh, the lug style talk is us guessing with some limited info, so don't get too worried about it yet.

Here's some more guessing: I wonder if your truck started life as a dually, or was it a single-wheel 3500 that someone converted with a kit? Next time you have a front wheel off, take a picture of the front hub area, please. Be interested to see what the spacer looks like. The ones on the rear have an aftermarket appearance, to me, anyway.

BTW you may already know this but your door panels are off a 90-94; 88-89 didn't have those style panels and window/lock buttons.

I was thinking something similar.

Is it the angle or is there next to no clearance for a socket on the lug nuts holding that adapter on?

Those drums look more like GM SRW drums than what I see on Rockauto for a 2000 E-350 van.

If you're turning a lot of RPMs you don't have high gears you've got low gears. Maybe you've got 4.56 to 1 gears, and we already know you don't have an OD gear with that TH400.

What size rims are those? 16" or something bigger?

It looks like a really good truck.

That is a gear ratio tag, indicating that it has 4.10 gears. Higher rpm means higher numerical gearing, also referred to as being "low geared". Confused yet? Lol.
What kind of bike have you been on?

I know it's a long shot on this old of a truck, but is the SPID label in the glovebox legible? You could post a pic, obscure the VIN if that matters to you. We want to see the RPO codes. Alternately, you could try running your VIN through Compnine or a friendly dealership if available; not all the older trucks come back with a report though so it could be hit or miss.

I'm pretty sure it's originally a dually, and still mostly sure the axle is too. I don't think the number stamped on the diff cover is is the BOM, and the 3 numbers at the bottom are a date.
And yes, my 88 had square buttons, so i was kinda guessing that wasn't original but hadnt paid it much mind.

Yep, 16 alcoas on it now, the blades are 16'' AR26s, and thank you!

Started on a Honda rebel and sold my car that same month, a year later moved up to a 2015 Harley FXDF and have been in love ever since.

So the glove box tag is long gone, if anybody does have the VIN hookup feel free, i'd love to see it.
