Motor stumbles while idling and light-mid loads, but not while in Park.

Ticking noise under acceleration/load

I've noticed recently that my Suburban is starting to tick under load or acceleration. Idle or cruise there is no ticking noise. Ive looked underneath to see if i might have a slight exhaust leak. Nope solid. I do have a lot of miles 237k, and I was hoping to keep on trucking till 250k+...
