I vote cough up for an NV4500; easiest to install, honestly if I were to get a manual truck I'd want one with a 4500. You know it'll mount up real nice and clean, and you won't have to dick around with aftermarket adapters, controllers, etc. etc.,

You'd be money ahead to wait for a complete running/driving Dodge to pull the 5.9 and trans out of. It'll take a while to find a deal on one because people think they're made of gold, but I have seen a decent deal pop up every now and then.

That block you have is sketchy. If I recall there was a big stink about the 5.9 Cummins when used in medium duty trucks like that wrecker, head gasket problems causing funky cylinder wear and...dammit my memory is fuzzy on this, so I'll take any corrections...seems like the cylinders are NOT sleeve design from OEM so repairing one gets expensive. I was looking into this years ago because we had a couple Freightliner FL60 rollbacks with 5.9's in them.

Planning and pricing for engine and transmission combos hits me right in the feels man. I've lost many a hour sleep on this

I want to do a 3126/Allison2400RDS and some transfer case I have yet to identify. Wheeeeeeeeeeeee

I say 12valve and NV4500. Do the KDP and refresh and drive it for now. Everything else can be added on later

I'd be lying if I said any option is cheap. @someotherguy has a good idea abt finding a donor truck, mayb someone who wrecked. Will take some time thou. If I remember right the cummins that got put into these bigger rigs were detuned to increase longevity since they usually drive 2-3 times as much miles as someone in a pickup. Best bet it to build it right the first time, it'll drain ur pockets and will take some time and some headaches but it'll save u in the end to get it right the first time and not have to redeal with it a later date.

That' what I was gonna say, diesel brothers put a 46re/47re behind a duramax in a square body. Honestly it'd probably be the easiest way to go atm.
theyre solid with $5k worth of work. until then they're just another notorious dodge tranny but they'll hold up for a good while behind a stock motor.

I did do some quick searching and 47res are definitely cheaper. And an 8.3 would be nuts, could talk **** on all the 5.9 guys st that point lol

Just put a big ole tranny cooler in there because iirc thats how the dodge granny burn up, the cooling system sucks and it torched them
